ROCKFORD - Hospital makes sure patients are taken care of after being discharged.
It's called the Med-to-Bed Program. Hospitalized patients who are close to being discharged will have the medication they need delivered to them.
"We will go to their room, double check what medications they are on, verify insurance coverage, process co-pays and deliver the medication right at the bedside" says Pharmacy Director Tom Carey. "Making sure patients go home on the correct medications is a big safety concern in health care nationwide."
SwedishAmerican also hopes the program will reduce the number of patients who are readmitted to the hospital. Some studies show that only a third of prescriptions written in the hospital are actually filled.
